Acceptable Documentation

The following are documents that are acceptable for documenting membership eligibility:

Eligibility	Acceptable Document(s)
Live	Driver License or other government issued ID containing a current address Bill or bank statement containing the name of the individual applying for membership Lease/rental agreement showing the address and name of the individual applying for membership
Employment	Current pay stub Letter from employer's HR/payroll manager
Worship	None required, however, name of church must be provided
School	Current/valid student ID Current schedule or tuition billing
Family	None required however name of sponsor member must be noted
